Tryout Check List: Must be complete before you arrive for tryouts
- 2011/12 USAV Membership Card (go to www.FloridaVolleyball.org to get your membership card. It must be done before you arrive for tryouts.
- 2011/12 AAU membership card
- Must bring a copy of your Birth Certificate to tryouts: You will not get this back.
- USAV Medical Release Form -Link Below. (Please make a copy of this for yourself. Your daughter will need to carry one with her at all time.)
